I agree to inform The Exercise Hub Ltd of any changes in my health status that may affect my safe participation in physical activities.*

To the maximum extent permitted under UK law (including the Unfair Contract Terms Act 1977), I waive any claims against The Exercise Hub, including staff, trainers, agents, or owners, for injuries or damages resulting from risks I have accepted. *

I understand this waiver does not exclude liability for negligence, serious negligence, or breach of statutory duty by the gym.

I agree to follow all gym rules, directions, class instructions, and maintenance procedures. I will report any defective or hazardous equipment immediately.

In the event of injury or illness, I consent to receive basic emergency first aid or medical treatment by qualified personnel, and I agree to bear any costs incurred.

I have undergone (or will undergo) an induction session where procedures, safety instructions, and correct use of equipment were explained by a staff member before starting activity.

Individuals aged 16 or 17 must have written consent from a parent or guardian. The parent/guardian must sign below and ensure that the minor understands the risks and obligations described in this document. Under 16's must have a supervising adult present
